Louise is right - see your doctor about the back pain. No-one can expect to sleep with pain.

Second bit of advice (once the back pain is under control) - you have to teach yourself to stop thinking about your worries in bed. I always think of flying a kite on a perfect, sunny winters day, and the feeling of it lifting me and landing me softly. (Yes it only happens in my dreams). But you've got to latch on to some thought that distracts you from any worries you have, and makes you feel happy and relaxed.
